Full name as it appears on the sacramental record

Approximate date of the sacrament (month/year is fine)

Your relationship to the person named (self, child, parent, etc.)

Purpose of the request (confirmation sponsor, marriage, RCIA, etc.)

Certificates are available only for sacraments registered at St. James the Less

Processing time is 3–5 business days

Suggested offering: Original certificates are free, copies are $15 per certificate
